The form identifies the deposit particulars and supporting deposit certificate to be supplied where a microorganism deposit is relevant to the invention.

- The application reference and the identity of the depositary authority and microorganism.
- The deposit number, deposit date and full address of the depositary authority.
- The deposit certificate issued by the depositary authority, together with the indication of whether it is attached or will be supplied within the period stated in the form.
